Apesar de ser uma das cirurgias estéticas mais realizadas, com recuperação rápida e resultados muito bons na grande maioria dos casos, o uso de próteses de silicone é fonte de polêmicas há muito tempo.
Na década de 90, os implantes foram proibidos nos Estados Unidos, devido à presença de moléculas de silicone em linfonodos (tecido de defesa) e possível associação deste achado com doenças imunes e câncer. A partir daí, as próteses foram aperfeiçoadas com a evolução de seu envoltório, para reduzir o extravasamento (microscópico) de silicone e outras complicações. Além disso, vários estudos científicos criteriosos demonstraram a segurança do procedimento, ocorrência similar de doenças (quando comparadas a pacientes sem prótese) e associação dos implantes com tumores de mama menos avançados, talvez devido ao maior número de exames de imagem ou à facilitação do autoexame.
Em 2004, ocorreu uma epidemia por Mycobacterium em próteses fabricadas no Brasil. Diversos pacientes precisaram de internação ou de remoção dos implantes. Houve diversas justificativas, incluindo o armazenamento inadequado dos implantes.
Em 2010, outro escândalo colocou em xeque toda a indústria das próteses: as francesas PIP, fabricadas com silicone industrial (impróprio para uso no corpo humano) e envoltórios de baixa qualidade com alto índice de rompimento. Este fato modificou a fiscalização de materiais médicos no Brasil e na Europa e destacou o valor dos critérios de qualidade adotados pelos americanos do FDA.
O assunto agora é o BIA-ALCL (sigla em inglês de Linfoma Anaplásico de Células Grandes Associado ao Implante Mamário). Esta doença é conhecida há alguns anos, com baixíssima incidência (ainda considerada raríssima) e associada à baixa agressividade. Porém, apresentou uma série de casos, prevalecendo pacientes usuárias de próteses Natrelle, da fabricante Allergan. Por este motivo, ocorreu recolhimento dos implantes ainda não utilizados na Europa, Estados Unidos e Brasil. Não existe qualquer recomendação para retirar as próteses em pacientes sem queixas (sintomas). Os sintomas que podem estar associados a este tipo de tumor são inchaço duradouro por acúmulo de líquido junto à prótese (seroma) e dor persistente, que podem ocorrer também em outras situações.
O que a portadora de prótese de silicone deve fazer?
- Identificar o seu implante (encontrar o cartão de identificação entregue na cirurgia ou solicitar cópia ao vendedor ou cópia do prontuário de internação);
- Realizar os exames periódicos de mama (assim como toda mulher deve fazer);
- Se apresentar os sintomas acima, procurar seu Cirurgião Plástico ou Mastologista;
- Acompanhar nos meio de comunicação novas informações, divulgadas pela ANVISA (Agência Nacional de Vigilância Sanitária) ou SBCP (Sociedade Brasileira de Cirurgia Plástica).
Não há motivo para alarme!
